Generate a printable or emailable document with all critical medical information. This feature is especially useful for sharing your medical history with new healthcare providers.

What does the TeamMed app do?

TeamMed allows you and your caregivers to manage your care in one place. It’s a comprehensive, user-friendly medical planning app that includes customizable features for tracking health and sharing your medical journey privately with your Support Circle.

Does TeamMed work for any medical condition?

Yes, TeamMed is designed to support any medical condition. Our users include individuals managing cancer, Crohn’s disease, epilepsy, diabetes, multiple sclerosis, and other chronic or rare illnesses. The app is customizable to meet your unique needs.

What can I track using TeamMed?

You can track almost anything related to your health. This includes symptoms, medications (with photos of prescription bottles), and vitals such as blood pressure and temperature. You can even track meal plans if needed.

How are my medical documents protected?

TeamMed uses secure third-party API, which meets or exceeds standard industry security measures and is HIPAA compliant, to protect your data, accessible to only valid users.

How do I use My Support Circle

My Support Circle is designed to help you stay connected with caregivers, friends, and family who want to support you. It allows you to ask for help with errands and other tasks and share updates through your newsfeed. You have full control over the permissions each supporter receives, tailoring access to fit your needs.

Do I need to pay for supporters in My Support Circle?

No, all supporters in your Support Circle are completely free to add and use.

What happens to personal and medical information if I delete the app?

If you delete the app, your account information will remain in the TeamMed database. To fully remove your data, you will need to delete your account through the app's settings.

How do I send or print a PDF of my information?

To generate a PDF, click the three dots next to the tracker or document you want to export. The PDF will be sent to your registered email address. If you want your supporters to view it in the app, just adjust their permissions.

How do I share the app with others?

You can easily invite others to use the app by selecting “App Invite” from the same screen where you add supporters. Just enter their name and send the invite.

How do I find feature instructions or videos?

Instructions and videos can be accessed directly through our website at www.teammedplanner.com/setup. Upon subscribing, you’ll also receive an email with a detailed list of features and links to videos for guidance.

How do I import documents and scans?

The simplest way to upload documents is by taking a photo or screenshot, which can then be added to the Documents feature. You can also export files from your provider's portal, save them on your device, and upload them to TeamMed.

How will I know when its time to renew my subscription?

We will send out an email to your email address on file two weeks before your yearly renewal date to remind you.

How do I cancel my subscription?

In the sidebar in the app, there is a section for manage subscriptions, and the option to cancel.

Our Journey

Susanne Riley, a medical professional, became a spouse-caregiver to her husband after he was diagnosed with an aggressive form of melanoma in February 2020 at age 36. Facing his cancer diagnosis and the COVID-19 pandemic simultaneously, she found it difficult to find a central platform to manage his medical needs.

Driven by a desire to control his care and provide a resource for other chronically ill patients, Susanne decided to create a Mobile App. She used her experience navigating cancer care, her work in the medical field, and advice from doctors to develop a tool to keep everything in one place.

Susanne brought in Lauren Steakley to assist with areas outside her expertise.

Lauren’s extensive experience in managing care for individuals with chronic health conditions, especially in home and private duty healthcare, has been crucial. Her profound expertise has been instrumental in crafting solutions that enable patients and caregivers to more effectively handle long-term care.

In collaboration with a dedicated development team, they have successfully crafted TeamMed's app to fit all of it's users needs.
